Rooted Care Institute was founded from lived experience and a belief that healing, dignity, and support should be accessible to everyone, especially those disproportionately impacted by HIV.
Carlos D. Howard, MHA, HIVPCP
Founder & Executive Director
Carlos D. Howard is a healthcare administrator, health equity advocate, and long-term HIV survivor. He is a doctoral candidate at Purdue University and seasoned leader in ambulatory healthcare operations, managed care, patient access, workforce leadership, financial performance, and healthcare compliance.
He founded Rooted Care Institute to bridge gaps between communities and trusted healthcare resources through education, navigation, partnerships, and support rooted in dignity, compassion, and equity. His work is driven by a deep commitment to ensuring that everyone, especially those disproportionately impacted by HIV, has access to the care, resources, and opportunities needed to live healthier, fuller lives.

His Story
As a long-term HIV survivor, Carlos' personal journey has shaped his commitment to prevention, stigma reduction, and culturally responsive care. His lived experience gives his a unique perspective on the challenges faced by individuals and communities, and it fuels his passion for helping others feel supported, valued, and seen.

Carlos brings extensive experience in healthcare administration, managed care, patient access, workforce development, operations, compliance, and financial performance. His career has been grounded in building high-performing teams, strengthening healthcare systems, and advancing equitable and community-centered solutions that improve health outcomes.

Rooted Care Institute was born from a desire to address persistent gaps in care and build stronger, healthier communities. Through prevention, care navigation, behavioral health support, and community education. Carlos is working to create a future where dignity, opportunity, and wellness are within reach for all, especially those most impacted by HIV.
