Rooted Care Institute advances health equity by providing integrated HIV prevention, PrEP education, care navigation, behavioral health, and wellness services to communities disproportionately impacted by HIV through compassionate, person-centered care that eliminates barriers, improves outcomes, and empowers lifelong well-being.

Rooted Care Institute is a community health navigation and education organization. We do not diagnose medical conditions, prescribe medications, or replace licensed medical or behavioral health providers. When clinical services are needed, we help connect individuals with qualified providers and community partners.
